Membership
- The membership scheme is open to both Adult and Child patients.
- An examination is required for all patients prior to joining the practice membership scheme.
- Payment is by monthly direct debit or annually by invoice.
- If the Direct Debit is cancelled within 12 months, then all treatment fees will need to be paid retrospectively for benefits already received.
- Terms and conditions are reviewed every 6 months and are subject to change.
- There is an initial registration fee that is payable with the first Direct Debit.
Cancellation
- If you ever wish to end the agreement you can do so by giving Denplan 1 months notice.
- If amounts due to us are not paid for 2 months in a row then we may revise the arrangement.
